- Type & nature of abuse being reported
- URL or Domain Name (if relevant)
- IP address on our network (if relevant)

- The exact location (URL) of each alleged infringing item (note that we can only act in respect of specifically identified content)
- A signed statement that you are the copyright holder or that you are authorised to act on behalf of the copyright holder.
- A signed statement that the report is accurate, is made in good faith and that you have sufficient reason to believe that you or your client’s copyright is being infringed.

- The exact location (URL) of each alleged infringing article or content item (note that we can only act in respect of specifically identified content)
- A signed statement that you are the the person being defamed, or are authorised to act on behalf of that person
- A signed statement that the report is accurate, is made in good faith and you have sufficient reason to believe that you our your client is being defamed
